Dear Parent and Candidate:
The following information is provided in preparation for the OMI Make-Up Parent Orientation on Wednesday, May 16, 2018, At 4:30 PM (Attendance by both the candidate and parent/legal guardian is the next mandatory step of the admissions process.)
Candidates must adhere to a prescribed uniform wear and appearance standard for the orientation (see attachment). An identification photo will be taken at the session.
If you have not turned in supporting documentation, please bring it with you. We will need the following supporting documentation:
- 1. Birth certificate or Passport,
- 2. Report card (grade 6, 7, and 8) Transcripts( grade 9, 10, 11, and 12)
- 3. Immunization record.
